North Monterey County Unified School District is committed to creating and maintaining a safe environment for our children to learn.  We need your help in gathering valuable information related to health, school safety, the learning environment, as well as alcohol, tobacco, and other drug issues. This information helps our district create better programs to help our students be happy, healthy, and safe at school.

Participate

Participation in the survey is voluntary and we hope you will give permission so that your child's opinion and knowledge can be part of the information we receive to make our schools better. We want to continue meeting the ongoing and changing needs of our students. The important information they provide us through this survey will help us do that.

The survey is anonymous. No names or any other identifying information is connected to the answers, except for the name of the school. To allow your child to participate, please follow the following steps according to your child’s grade level.

Thank you for your continued support to ensure we provide safe and supportive learning environments for all students.

If you wish to view the survey samples, you may do so by clicking on the links below or by clicking your students' grade level. Copies of survey samples will also be available at the main office at each school site. 

Elementary Grade Parents: To allow your child to participate, please complete the permission slip provided by your child's teacher and return it to your child’s school. We need your permission for elementary students to take the survey.

Middle School Grade Parents: Please note that the CHKS survey for middle school students requires passive consent, which means you only complete the form if you do NOT want your child to participate.

High School Grade Parents:  The CHKS survey for high school students requires passive consent, which means you only complete the form if you do NOT want your child to participate.
