In order for schools to administer the required medication for your student, please complete the Permission to Administer Medication Form. A NEW form must be completed EVERY school year for EACH medication, as well as whenever there is a change in the pupil's authorized health care provider, the medication dosage, the method by which the medication is required to be taken, or date(s) or time(s) the medication is required to be taken.
