    Transcripts for Current NMCHS Students 
    All transcripts must be requested from the school registrar 24 hours in advance by signing a Transcript Request Form. Allow two school days for processing. 


    Transcript Request for Former NMCHS Students
    Mail or drop off a money order addressed to NMCHS to the address below along with the Transcript Request Form and a copy of valid photo ID:

    The following fees apply: 
    Unofficial Copy: $3.00 per copy
    Official: $5.00 per copy
    Official – Rush Processing: $10.00 per copy (processed day of receipt). 


    Graduation Verification for former students
    ** High School Diploma Verification Dates must be requested in writing. Please fax to 831-453-2009

    Withdrawal Information
    Students needing to withdraw from school for any reason must report to the Registrar's Office accompanied by a parent or guardian to sign a Student Withdrawal Form.
    Students are required to complete the following:

    1) Obtain withdrawal grades from teachers.
    2) Return all books and any school items on loan
    3) Checkout at designated offices (i.e. Attendance, Athletics, Finance, and Career Center).

    Once completed, a copy of the withdrawal form, withdrawal grades, and transcript will be provided to the parent/guardian.
